The article examines the management of acute respiratory infections (ARI) at the Children's Hospital in Islamabad, Pakistan. During the period 1989-1992, antibiotic use in the outpatient department of the hospital decreased. Also, the case fatality rate in children admitted with ARI declined from 9.9 percent to 4.9 percent. The hospital's standard case management for ARI reduced the use of antibiotics and drug expenditures. The case management strategy also contributed to the significant decline in the fatality rate among children with ARI.
